Prescription Refill Policy
Prescription refills may be requested by calling our office during regular business hours. Approved refill requests will be called in or faxed to your pharmacy within 48 hours.

Cancellation Policy
We require at least 24 hours’ notice to cancel or reschedule an appointment with a physician or a member of our medical staff. Appointments canceled with less than 24 hours’ notice may result in a $30 fee. Providing advance notice helps us offer appointment times to other patients who may need care.

Financial Policy
We accept most major insurance plans listed by our office, including Medicare and Medicaid.  While many secondary or supplemental plans automatically cross over from Medicare, it is the patient’s responsibility to confirm that the “automatic crossover” is in place by contacting Medicare and the secondary or supplemental plan. Any patient-responsible balance is due at the time of service. We accept cash, check, debit cards, and major credit cards, including Visa, Mastercard, and American Express.
